La solicitarea biletului de tratament sanatorial poate fi anexat încă un document

Solicitanții biletului de tratament sanatorial, după caz, prezintă certificatul eliberat de autoritățile fiscale prin care se atestă desfășurarea sau lipsa activității de întreprinzător pentru ultimele șase luni premergătoare lunii eliberării biletului. Serviciul Fiscal de Stat propune spre consultare publică proiectul Ordinului cu privire la aprobarea Certificatului privind desfășurarea sau lipsa activității de întreprinzător pentru ultimele șase luni premergătoare lunii eliberării biletului de tratament. Documentul a fost elaborat în temeiul prevederilor pct. 21 lit. e) din Regulamentul privind condițiile, modul de asigurare, evidență și distribuire a biletelor de tratament sanatorial acordate veteranilor (aprobat prin Hotărârea Guvernului nr. 190 din 17 martie 2010), precum și conform prevederilor statuate de art. 1323 pct. (10) din Codul fiscal (exercitarea atribuțiilor de către conducerea SFS). Amintim că, conform reglementărilor stabilite prin HG nr.190, la eliberarea biletelor de tratament organele abilitate (CTAS) pot solicita de la beneficiar și certificatul eliberat de autoritățile fiscale prin care se atestă desfășurarea sau lipsa activității de întreprinzător pentru ultimele șase luni premergătoare lunii eliberării biletului. În certificat se vor indica datele solicitantului (numele și prenumele contribuabilului, adresa, codul fiscal) și conformarea informației solicitate. Totodată, se propune ținerea evidenței certificatelor eliberate într-un Registru de evidență, în care se va nota numărul certificatului, numele și prenumele solicitantului, codul fiscal, numărul de intrarea și data cererii, data eliberării certificatului și executorul. Pentru ridicarea Certificatului privind desfășurarea sau lipsa activității de întreprinzător pentru ultimele șase luni premergătoare lunii eliberării biletului de tratament solicitantul urmează să se adreseze la subdiviziunea structurală a SFS.
